National Healthcare Decisions Day

Westchester End-of-Life Coalition is participating in a national effort to promote advance directives. The Coalition will have trained volunteers stationed at libraries across Westchester County to explain the New York State Health Care Proxy. The volunteers will hand out Health Care Proxy forms free of charge and provide assistance to anyone who needs help filling out the forms.
